術中経食道心エコーで発見した可動性のcalcified amorphous tumor

Mobile Calcified Amorphous Tumor Detected by Intraoperative Transesophageal Echocardiography

要旨

　大動脈弁置換術の術中経食道心エコーにおいて，大動脈弁に可動性の腫瘤性病変を発見し，摘出標本の病理検査でcalcified amorphous tumor（CAT）と判明した症例を経験した。CATは透析患者や僧帽弁輪石灰化（mitral annulus calcification：MAC）を有する患者に合併する頻度が高く，もし手術予定部位以外の心腔内にCATを示唆するような可動性の腫瘤を発見した場合に塞栓症予防や確定診断のために外科的切除を行う可能性があるので，透析患者やMACを有する患者の管理では短期間で発症するCATを念頭に置いて経食道心エコー（TEE）を評価することが重要である。

　We report a patient with a mobile mass of the aortic valve detected by transesophageal echocardiography during aortic valve replacement. Pathological examination of the isolated preparation revealed a diagnosis of calcified amorphous tumor. It is important to evaluate by transesophageal echocardiography considering the possibility of detecting new mass lesions that have not been previously diagnosed.
