骨間筋の部分切除を要した手背部AVMの1例

A Case of AVM in the Dorsal Hand Required Partial Resection of the Dorsal Interosseous Muscle

はじめに

　動静脈奇形（arteriovenous malformation：以下，AVM）の治療としては塞栓術，硬化療法，手術，これらの組み合わせなどさまざまな選択肢があるが，AVMはいまだに治療の困難な疾患の1つである。今回われわれは手背部のAVMに対して骨間筋の部分切除を伴うAVM切除術を行い，良好な結果を得た1例を経験したので報告する。

　The treatment of extracranial arteriovenous malformations (AVMs) is still challenging because of the complex vasculature and involvement of surrounding tissues such as nerves, bones, muscles, and tendons. The treatment of AVMs in the hand require special attention to these tissues in order to preserve postoperative functions and sensations. We present the case details of a 50-year old woman with an AVM in her left dorsal hand which was successfully treated with surgical resection at a single session. Despite the combined resection of parts of the 1st to 3rd dorsal interosseous muscles, postoperative functional impairment was not observed. Although embolization or sclerotherapy can be options in the treatment of a resectable AVM, surgical treatment has advantages for AVMs in the hand region.
