ミラドライⓇを用いた腋窩多汗症・腋臭症治療（2）—合併症とその対策—

Treatment of Axillary Hyperhidrosis Using a Microwave-based Energy Device, the MiraDry Ⓡ: Complications and their Preventions

はじめに

　腋窩部の汗（いわゆる，わき汗）やにおいに悩む人は多い。従来からの外用薬と手術法に加えて，2012年にA型ボツリヌス毒素の局所注射療法が，2021年と2022年には2つの抗コリン外用薬が相次いで発売された。ミラドライ Ⓡはマイクロ波を用いた原発性腋窩多汗症治療機器で，本邦では2018年6月に本邦の薬事承認を取得したが，その使用実績はすでに10年を経過している。現在，腋窩多汗症・腋臭症の治療は選択肢が増え，外用剤，ボトックス注入療法，手術，ミラドライ Ⓡなどから選択が可能である。われわれの施設ではミラドライ Ⓡ導入5年目を迎え，その症例数は350例を超えた。本稿では，より治療効果を高めるためのわれわれの工夫，および合併症とその予防・対策について焦点をしぼり述べる。

　The use of the miraDry Ⓡ microwave-based energy device is an effective treatment for axillary hyperhidrosis and axillary osmidrosis. The miraDry Ⓡ received pharmaceutical approval in Japan in 2018. We use the miraDry Ⓡ with 2 PASS irradiation at areas approx. Potential complications of this treatment include brachial plexopathy, cutaneous neuropathy, burn scarring, and abscesses. The tumescent method using a large amount of local anesthetic is effective for the prevention of brachial plexopathy and cutaneous neuropathy. Abscesses are thought to be related to the presence or absence of hair. When used correctly, the miraDry Ⓡ is a safe and effective treatment for axillary hyperhidrosis and axillary osmidrosis.
