ビューホットⓇを用いた腋窩多汗症・腋臭症治療

Treatment of Axillary Hyperhidrosis and Osmidrosis with Fractional Needle Radiofrequency Waves （View HOT）

はじめに

　腋臭症治療や多汗症治療において剪除法に代表される手術療法の治療効果は高いが，合併症を起こさないように施術するためには術者の高い習熟度が求められる。それを補うために，吸引法 1）や，超音波破砕吸引 2），シェーバー 3）などの特殊手術機器の使用がなされてきた。近年では合併症率が低いうえに治療効果や満足度の高い非手術の治療法のニーズが高まり，マイクロ波やラジオ波を用いた治療機器が開発されるようになった。著者は2014年からラジオ波を用いたビューホット Ⓡによる治療に携わってきた。術者による偏りが少ない機械で特殊な技術がなくても施術可能であるが，いくつかの注意点や治療上のコツがある。本稿ではそれらを解説するとともに，2019～2020年にかけて低侵襲H&O臨床医学研究会によって行われた多施設共同研究の結果も紹介する。

　There is an increasing need for treatments for axillary hyperhidrosis and osmidrosis that are both highly effective and have a short downtime. I have been involved in the provision of View HOT treatment since 2014 and have also created View HOT treatment protocols. The View HOT is a device that destroys sweat glands with radiofrequency （RF） waves. At one irradiation, 36 needles pierce the skin surface, and RF waves are emitted from the tip of each of the needles; the heat emitted burns the apocrine glands and eccrine glands. The procedure is repeated throughout the axilla. This paper describes （i） the transition of View HOT use and its protocol, and （ii） the treatment results obtained with the View HOT in multicenter joint research by the Minimally Invasive Hyperhidrosis Axillary Odor Treatment Study Group.
