Pitfalls in Managing Buccal Subcutaneous Tumors

はじめに

　近年，皮膚・皮下腫瘍で特に顔面に生じたものでは，その多くが形成外科に紹介されてくる。したがって，形成外科医は顔面の解剖とその部位に生じる腫瘍について熟知しておくことが必要である。さらに顔面の中でも頬部には，皮膚や脂肪組織だけでなく耳下腺や顔面神経およびリンパ組織などが存在し，それらの組織から多種多様な腫瘍が発生してくる可能性がある。したがって，これらの腫瘍を術前に鑑別診断することは，手術を行う形成外科医にとって非常に重要である。しかし，それらの臨床所見やCTおよびMRIなどの所見は非常に似通ったものが多く，最終的には摘出物の病理診断によって診断が確定されることも多い。本特集では，頬部に生じた皮下腫瘍の診療を行ううえでのピットホールについて述べる。

　To avoid pitfalls in the management of buccal subcutaneous tumors, it is important to always keep in mind that （i） various types of subcutaneous tumor occur in the cheek, and （ii） subcutaneous tumors that occur in the cheek may be pleomorphic or malignant even if they are located far from a parotid gland. It is also necessary to be completely familiar with the anatomy of the accessory parotid glands and to perform the tumor-removal operation carefully, especially for pleomorphic tumors arising in an accessory parotid gland. Damage to the facial nerve and capsule of the tumor must be avoided.
