巨大な局所進行乳癌原発巣に対し一次治療として放射線治療を行った1例

A case of radiotherapy as temporary treatment for a giant locally advanced primary breast cancer

局所進行乳癌への治療はまず全身薬物療法を行い，続いて局所療法（手術，放射線治療）を行うのが標準的である^1）。局所進行乳癌への放射線治療の位置づけは，薬物療法，手術後の術後照射か，手術不能例に行うとされており，一次治療としての乳癌原発巣への放射線治療は一般的ではない。今回我々は，全身状態低下のため全身薬物療法が困難であった巨大局所進行乳癌に対し一次治療として放射線治療を行い，その後に薬物療法，手術を行うことができた症例を経験したので報告する。

The standard treatment for locally advanced breast cancer is systemic drug therapy followed by local therapy（surgery and radiotherapy）. Radiotherapy for locally advanced breast cancer is typically administered as adjuvant therapy after drug therapy or surgery, or as primary therapy for inoperable cases. However, radiotherapy to the primary breast tumor itself is not commonly used as primary therapy. We report a case where we administered radiotherapy as primary treatment for a large locally advanced breast cancer that was difficult to treat with systemic drug therapy due to poor general condition. Subsequently, we were able to perform drug therapy and surgery.
