画像上で筋腫大を主体とする筋骨格病変を呈した全身性アミロイドーシスの1例

A case of systemic amyloidosis presenting with predominant musculoskeletal involvement and muscle enlargement on imaging

アミロイドーシスとは，アミロイドと呼ばれる異常な線維状蛋白の沈着により臓器障害が引き起こされる疾患群である。その病型の1つであるALアミロイドーシスは異常形質細胞から産生される単クローン性免疫グロブリン軽鎖に由来するアミロイドの沈着を原因とし，原発性として発症するほか，形質細胞腫瘍である多発性骨髄腫を背景に発症することも知られている^1）。ALアミロイドーシスによる臓器障害は腎臓，心臓，消化管に多く，筋病変はまれとされている^2）。今回我々は，筋腫大を主体とする筋骨格症状と多発溶骨性病変を呈して多発性骨髄腫に合併したALアミロイドーシスと考えられた1例を経験したため，文献的考察を加えて報告する。

We report a case of AL amyloidosis associated with multiple myeloma presenting with predominant muscle involvement. A woman in her 40s developed progressive proximal muscle enlargement and weakness. Imaging studies demonstrated symmetric muscle enlargement and additional lesions in the temporomandibular joint suggestive of amyloid deposition on MRI. This case is noteworthy for the imaging evaluation of musculoskeletal manifestations of AL amyloidosis with predominant muscle involvement.
