卵巣線維腫症との鑑別に苦慮した乳癌卵巣転移の1例

A case of breast cancer metastasis to the ovary mimicking ovarian fibromatosis

乳癌原発の転移性卵巣腫瘍に関するまとまった画像所見の報告は少ない。病理学的には辺縁に線維成分が偏在し，中央部に浮腫性変化を示すことが特徴の1つとされる。今回我々は，MRI T2強調像にて卵巣線維腫症に特徴的とされるblack garland-like appearanceを呈し，鑑別に苦慮した浸潤性小葉癌の卵巣転移例を経験した。本所見を示す乳癌由来の転移性卵巣腫瘍の報告はまれであり，文献的考察を加えて報告する。

Metastatic ovarian tumors originating from breast cancer may exhibit solid components predominantly located at the periphery, making differentiation from ovarian fibromatosis. We report that approximately 6 cm masses were noted in both ovaries, and T2-weighted images showed a band-like low signal at the margins, resembling ovarian fibromatosis. Pathological diagnosis confirmed metastatic ovarian tumors originating from a 12-year-old breast cancer（invasive lobular carcinoma）. Ultimately it was difficult to make a determination based solely on imaging findings. It is important that it shows a pattern similar to the black garland-like appearance.
