直腸癌の術前診断で手術が施行された粘膜脱症候群の1例

A case of mucosal prolapse syndrome resected with preoperative diagnosis of rectal cancer

粘膜脱症候群とは直腸粘膜の一部が繰り返し脱出し，潰瘍性病変や隆起性病変などを形成する良性疾患であるが，直腸癌などの他疾患との鑑別を要する場合がある。粘膜脱症候群を直腸癌と診断し，人工肛門造設といった不要な治療がなされるおそれがあり，画像診断で粘膜脱症候群の可能性を示すことが重要である。

A 43-year-old man presented with a prolapsed anal mass. Biopsy revealed Class Ⅴ adenocarcinoma. MRI showed a 3-cm pedunculated tumor protruding from the anus, with mixed signal intensity on T2-weighted images. No metastasis was detected on CT. Suspecting mucosal prolapse syndrome, a conservative excisional surgery was performed. The pathological diagnosis of mucosal prolapse syndrome was made by surgical specimen.
