Radiomics and radiogenomics offer a paradigm shift in lung cancer care, converting medical images into high-dimensional data to non-invasively decode tumor phenotypes and genotypes. This “virtual biopsy” approach aims to predict prognosis, treatment response, and genetic mutations like EGFR. However, clinical translation is hampered by a “reproducibility crisis” and a “translation gap.” This article details the workflow, clinical applications, and critical initiatives like IBSI and TRIPOD+AI that address these challenges. Furthermore, it explores the transformative potential and emerging complexities of foundation models, which are reshaping the future of integrated diagnostics and the radiologist’s role.
