放射線治療後再発に対する緩和的動注療法

Salvage transarterial chemotherapy is an effective palliative treatment for advanced recurrence post-radiotherapy

がん治療は局所治療と全身治療に大別される。放射線科医が担当する放射線治療，IVRはいずれも前者に該当するが，再発がんおよび転移性がんの治療の主役は後者である。そして分子標的薬，免疫治療薬，さらに最近は抗体薬といった薬物治療の開発が劇的に進み，局所治療に対するがん治療医の関心は低くなった。動注療法に関する現状と保険上の制限などに関しては，『IVRのすべて』^1）の中で詳細にまとめられているのでぜひ一読していただきたい。当科で動注療法に使用している抗がん剤の多くがoff-labelであり，医療コンプライアンスの厳しい現在においてその実施自体も難しいことを念頭に，本稿では，当科ではどのように治療しているか，動注の適応，そして今回の主題である照射野内再発に対する当科の動注の成績について紹介する。

The need of transarterial chemotherapy has been reduced in the current cancer treatment system due to the lack of evidence. In this article, we describe the indications and current status of our department of transarterial chemotherapy, as well as the results of our use of transarterial chemotherapy for patients with recurrence in the irradiated field who are unable to receive additional irradiation due to late side effects. Transarterial chemotherapy should be further recognized as a salvage pharmacotherapy and palliative IVR.
