Treatment of resectable lung cancer has historically relied on the TNM classification, and the standard of care has traditionally been lobectomy plus lymph node dissection and adjuvant treatment with tegafur uracil or platinum-based chemotherapy. The 2020 ADAURA trial demonstrated the efficacy of osimertinib as adjuvant chemotherapy for patients with EGFR-mutant lung cancer. Clinical trials including JCOG0802/WJOG4607L and CALGB140503 demonstrated efficacy of sublobar resection for small NSCLC. Accurate patient classification has been shown to improve precision surgery outcomes. Although regional differences in approaches and patient populations between Japan and Europe and the United States need to be considered, sublobar resection, including wedge resection and segmental resection, is becoming the standard for small lung cancer. These findings led to a paradigm shift, driving individualized therapeutic strategies beyond TNM.
