PETを用いた転移病変の評価：FDG，PSMA，FAPI

Evaluation of metastatic lesions using PET：FDG, PSMA, FAPI

近年，従来のブドウ糖類似体のFDG（フルオロデオキシグルコース）を用いたFDG-PETに加えて，さらに検出感度の高い新たなPETプローブが登場し，臨床で広がりつつある。FDG-PETは様々な癌種で幅広く利用できる一方で，前立腺癌など一部の癌では集積が低いことが多く，また胃癌・膵癌においても低集積を呈する場合があることが知られている^1）。

In recent years, new PET probes with higher detection sensitivity have been developed other than conventional FDG. Among them, PSMA-PET has high detection sensitivity for recurrent and metastatic lesions of prostate cancer, and it has already been demonstrated that oligometastases can be detected with high accuracy, and local therapy, such as RT or surgery, can achieve the excellent disease control. FAPI-PET is also a promising PET probe which has good detection sensitivity in many malignant tumors, especially in pancreatic and gastric cancers. Detection of oligometastases by FAPI-PET is expected to have the potential to make a significant impact on treatment strategies similar to PSMA-PET.
