実はこれまで急性腹症といえば内因性（非外傷性）病態を指すものと思い込んでいた。本稿の執筆依頼をいただいた際，与えられた「外傷における急性腹症」というテーマを見てはっとした。外傷に伴って腹膜炎や腸閉塞を生じうることや，外傷のCTを読んでいる際に（いわゆる内因性の）急性腹症CTを読むときのような画像所見の捉え方が役に立つ場面があることを思い出したからだ。急性腹症ガイドライン^1）による急性腹症の定義においても，非外傷性とは限定されていない。このような実践的なテーマについて述べる機会を与えていただいたことに感謝するとともに，本稿を目にしていただいた読者の方々に，通常の外傷画像診断の特集記事とはやや趣の異なる新鮮さを感じていただければ幸いである。筆者らが外傷画像診断に携わるうえで日頃気をつけていること，pitfallやまれな病態を含め実際の症例を通して学ばせていただいたことをcase reviewの形でお伝えしたい。

The acute abdomen may be often recognized as a non-traumatic disease in daily practice. This time, we present and explain the acute abdominal diseases related to trauma that are educational, including bowel injury, main pancreatic duct injury, traumatic abdominal wall hernia, infected retroperitoneal hematoma and bladder penetration, necrotizing soft tissue infection, and so on. To detect accurately the acute abdomen due to trauma, we should evaluate whole-body CT in trauma from the point of view of diagnosing the acute abdomen, following diagnosis and treatment of initial traumatic injuries.
