腹部画像診断において膵癌を正しく診断することは極めて重要であるが，逆に膵癌ではないものを正しく膵癌ではない他の疾患であると診断することも侵襲の大きな膵臓の手術や化学療法を安全に避けることにつながり，極めてその臨床的意義の大きなものである。本稿では膵癌と鑑別を要する疾患というテーマで，特に重要と思われる疾患についていかにして「膵癌らしくない」，またより「その他の疾患らしい」を判断していくかを解説していく。

There are a wide variety of diseases that need to be differentiated from pancreatic cancer, and it is very important to correctly diagnose these diseases as not being pancreatic cancer. In this article, we will present primary pancreatic tumors, secondary pancreatic tumors, inflammatory pancreatic diseases, and peripancreatic tumors that need to be differentiated from pancreatic cancer and describe what findings were the key to differentiate them.
