Hematopoietic active bone marrow usually exists in the vertebrae, to which sometimes malignant tumors metastasize. Although FDG-PET is useful to visualize malignant tumor lesions, the tracer distribution does not always indicate neoplasm. Especially, the accumulation in the vertebrae reflects the hematopoietic function;therefore, physiological condition affects and changes the FDG-PET images. This paper explains the factors which influence the bone marrow physiological accumulation and the suspected mechanism causing “bone pseudometastases”, which means localized hyperplastic hematopoietic active marrow mimicking bone metastases.
