Accessory cavitated uterine mass（ACUM）は正常子宮に発生する嚢胞性腫瘤で，ミュラー管の発生異常が原因と推察されている比較的新しい疾患概念である。症状は重症の月経困難症や慢性的な下腹部痛であり，30歳未満の若年女性に好発する。従来，若年性嚢胞性子宮腺筋症と診断されていた症例の多くはACUMであるとされ，子宮腺筋症とは異なる病態と考えられている。

Accessory cavitated uterine mass（ACUM）is a rare form of Müllerian anomaly which present severe dysmenorrhea and recurrent pelvic pain in young females. ACUMs have been reported differential diagnoses such as juvenile cystic adenomyoma/adenomyosis but they are not adenomyosis. We present a 14 years old female with severe dysmenorrhea who underwent CT/MRI and proved by surgical pathology. Early surgical excision of the mass could prevent the prolonged suffering of these young women. The entity is not as infrequent as previously thought, emphasizing the importance of understanding of the radiologic features and making a correct diagnosis.
