Practical use of Ra223 treatment：real world of Xofigo® therapy for prostate cancer

前立腺癌の増加傾向は他のがん腫に比べて著しく，その理由として高齢化社会への変遷と前立腺特異抗原（prostate specific antigen：PSA）による検診の普及によると考えられている。以前に比べると，検診で発見される早期癌の割合は多くなったが，排尿障害や骨痛などの臨床症状で来院したりするような進行癌の症例も一定の割合で存在する。前立腺癌は進展すると骨転移をきたすのは周知のごとくであり，初期ホルモン療法が無効になった場合，つまり去勢抵抗性前立腺癌（castration resistant prostate cancer：CRPC）では骨転移の増悪が問題となる。骨転移部位の骨痛などによってQOLは低下し，病的骨折やそれに対する手術，あるいは放射線治療などの症候性骨関連事象が生じてくる。このようなCRPCの骨転移に対して，α線のRa223を用いた放射線治療が開発され，我が国においても普及してきた。このRa223治療の実臨床について解説する。

It is well known that the most frequent sites of metastasis in patients with advanced prostate cancer is bone with osteoblastic change. Most castration resistant prostate cancer（CRPC）patients suffer from bone metastasis. Radium223（Ra223）dichloride emits α particles and has been proven to improve overall survival and QOL by reducing symptomatic skeletal symptoms in metastatic CRPC patients. In practice, the timing of Ra223 treatment has become important because there are several CRPC therapies such as hormonal or cytotoxic agents. The completion of 6 cycles of Ra223 is critical for prolonging OS. The optimal timing of Ra223 administration is recommended the duration prior to chemotherapy.
