FFRCTによる虚血診断の実力―光と影

Diagnostic performance of FFRCT for detecting hemodynamically significant coronary artery disease in the clinical setting

コンピュータを用いて冠動脈CTのデータからFFRを算出する解析法（computed tomography derived fractional flow reserve：FFR_CT，HeartFlow^®）^1）の保険点数が償還され, 2018年12月1日より我が国でも実臨床においてFFR_CT解析が行えるようになった。これにより冠動脈CT検査のみで形態的評価と機能的評価を一元的に非侵襲的に行うことが可能であり，侵襲的冠動脈造影検査（coronary angiography：CAG）や経皮的冠動脈形成術（percutaneous coronary intervention：PCI）の適応を判断するうえでのゲートキーパーとしての有用性が非常に期待される最新の解析方法である。本稿ではFFR_CTの有用性（光）と臨床使用における問題点（影）について概説する。

Recently, new technology of FFR_CT（HeartFlow^®）for non-invasively calculating fractional flow reserve derived from coronary computed tomography has come to be covered by insurance and has be attracted attention. FFR_CT can perform morphological and functional evaluation in one modality non-invasively and has high diagnostic sensitivity negative predictive value, it is therefore FFR_CT is considered to be useful for screening coronary artery lesions and is expected as a gatekeeper for catheterization/PCI. We present the clinical usefulness of FFR_CT from the clinical evidence and our experience, and consider the possibility of change in the clinical diagnosis flow by FFR_CT.
