非造影CTによる腹腔動脈解離・上腸間膜動脈解離の診断

Diagnosis of celiac/SMA dissection on non-contrast enhanced CT

川崎幸病院救急科は年間約1万人の救急搬送患者と年間約2万人のwalk-in患者を受け入れている。CTに関する患者緊急度は，造影CTを至急で行うべきと医師が判断する患者群から非造影CTで一応スクリーニングしておこうと判断される患者群まで様々である。医師の造影剤に対する考えは，積極的に投与する医師からそうでない医師まで様々である。既往歴や腎機能が不明のため，まず非造影CTを撮影することも少なくない。本院に併設されている4つの一般外来クリニックでも，患者重篤度や医師の造影剤に対する考え方は様々である。川崎幸病院と併設4クリニックでの2017年のCT総件数40,055件のうち造影検査は17.2％である。併設4クリニックの画像診断は川崎幸病院で行っている。

Imaging findings of a dissection of celiac axis or SMA on non-contrast enhanced CT were retrospectively reviewed. Seventeen patients were suspected to have a dissection at routine clinical practice. Finally, eleven patients were diagnosed to have a dissection. Three findings（arterial dilatation, intraarterial hyperdense sign, periarterial fat stranding）were noted. Cases which had two or three findings should be diagnosed to have a dissection.
