TKA周囲大腿骨骨折で大腿骨コンポーネントにゆるみがなく，bone stockが保たれている場合は，ロッキングプレートもしくは逆行性髄内釘による固定が行われる。本稿では，逆行性髄内釘を行う際の注意点，および近年の話題について述べる。
