高所からの墜落外傷では脊椎骨折に胸部大動脈損傷を合併することがある．胸部大動脈損傷は医療機関到着前に死亡する場合が多いが，血腫による一時的な止血や仮性瘤の形成で循環動態が保たれている場合があり，胸部や肺にまったく損傷がない症例もある．われわれは，2014年9月～2016年8月の2年間で胸椎，腰椎の骨折に大動脈損傷を合併した3例を経験したので報告する．
