Cannulation Techniques for Cardiopulmonary Bypass During Cardiac Surgery

カニュレーションは心臓大動脈手術の手技の中で，はじめて心臓や大動脈に針糸を入れ，穿刺や切開を行う作業であり，板前でいうと，追い回しを卒業し，はじめて包丁を使って作業するようなものである．施設や外科医によって決まった手順で行われることが多く，その決まりには大した論理的根拠がない場合も少なくない．しかしながら，安全に手術を行い，針刺し事故を未然に防ぐ意味でも，術野のメンバー全員がその手順を共有していることが望ましい．よって，修練医師はアテンディング医師それぞれのやり方を取得し，メンバー全員がアテンディング医師に合わせた手順で手術をすすめていくことが必要となる．

Cannulation for cardiopulmonary bypass is a fundamental procedure in cardiac surgery. Different surgeons perform it in a different way without any scientific reason, and therefore, it is extremely important that the whole surgical team remember and follow the routine of each attending surgeon to avoid confusion or needlestick accidents. In this section, the author’s routine cannulation technique is described.
