◆要 旨◆
【目的】入院中に看取りとなった利用者とその家族の情報を記載した看護情報提供書を訪問看護師へ提供することの有用性を明らかにする.
【方法】2013年から2014年の期間に,看護情報提供書を送付した全42施設に所属する訪問看護師に対し,質問紙調査を行った.調査項目は,〈看護情報提供書の活用性〉,〈訪問看護師が必要とする情報〉,〈訪問看護師が求める情報提供体制〉とした.分析は,記述統計を用いた.
【結果】〈看護情報提供書の活用性〉では,「自分たちの看護を振り返る機会になると思う」,「利用者の最後の様子を知ることで自分たちの看護が完結することを実感できると思う」など,〈訪問看護師が必要とする情報〉では,「入院のきっかけとなった症状は緩和されたのか」,「利用者の思いに沿った最期を迎えることができたか」などの項目が順に高かった.〈訪問看護師が求める情報提供体制〉では,「連絡の手段」は手紙が51.1%でもっとも多く,「情報記載量」はA4用紙1枚程度が73.3%であった.
【結論】看護情報提供書の送付は,看護の振り返りや,入院中から看取りにいたるまでの利用者とその家族の様子に関する情報提供に有用であることが明らかとなった.
This study aimed to elucidate the utility of providing a nursing report of the cancer patient and their family to the visiting nurse who had been caring for the cancer patient before he or she was hospitalized in a palliative care unit where he or she passed away.
We conducted a questionnaire-based survey among visiting nurses who were working for 42 facilities that sent nursing information reports between 2013 and 2014. Survey items included 1)Utilization of nursing information reports, 2)Information that visiting nurses need, and 3)Information providing system that visiting nurses want. +Data analysis was performed using descriptive statistics.
The most frequent responses to first topic were “it can be an opportunity to review nursing care that I provided,” followed by “I can feel the conclusion of my nursing care service by knowing the last stage of the patient.” The leading responses to second topic were “whether symptoms that required hospitalization were relieved,” followed by “whether the last stage of the patient met his perception.” The most frequent responses to third topic were 'letters,’ and 'one page on A4 size paper.’
This study reveals that sending nursing information reports is useful for reviewing the nursing care performed by visiting nurses and providing information about the patient and their family from hospitalization to the end of life.
