 Purpose: The purpose was to understand the association between actual defecation self-management performed by patients themselves and preferences for guidance in order to use it as basic data for providing guidance to patients based on their defecation status.

 Method: We conducted a self-administered questionnaire survey with 800 patients who visited the outpatient department of A university hospital between December 2013 and February 2014.

 Results: Of the valid responses from 559 subjects on their preferences for guidance regarding defecation, 34.7% responded instructions on how to take medication, 44.2% responded instructions on how to exercise, and 51.5% responded instructions on food intake. We performed secondary analysis on the association between the four selfmanagement factors pertaining to defecation reported in a previous study (Yamahata et al., 2016) and preferences for guidance. With regard to the association between self-management factors and preferences for guidance, the high “drug utilization” group preferred guidance on how to take medication (OR = 1.93, p < 0.01), how to exercise (OR = 1.71, p < 0.01), and food intake (OR = 1.69, p < 0.05) in comparison to the low “drug utilization” group. The actual performance statuses of “diet management” and “exercise habits” were not associated with a preference for guidance.

 Conclusion: Those using drugs as part of self-management preferred guidance. Although those with issues regarding lifestyle habits require guidance, the actual performance status of an individual was not associated with a preference for guidance. We need to encourage patients in a way that motivates them in accordance with actual self-management practice.
