3．慢性腎臓病における患者教育プログラムの経済性に関する文献研究

　【目的】本研究の目的は，慢性腎臓病における経済性の優れた患者教育プログラムを見出すために，プログラムの目的，内容と実践，患者への効果，経済性について明らかにすることである．【研究方法】研究方法は文献研究である．医学中央雑誌Web版を用いて検索し，キーワードは“患者教育”，“血液透析”，“経済学”とし，論文の種類は“原著論文”とした．PubMedではキーワードを“patient education”，“renal dialysis”，“economics”とした．そのなかでプログラムの内容が記載されている文献のみをレビューの対象とした．【倫理的配慮】本研究は文献研究であるため，倫理的配慮は該当しない．【結果・考察】わが国では，慢性腎臓病における経済性の優れた患者教育プログラムはなかったが，海外では6編の報告があった．プログラムの目的は，在宅血液透析の実践と継続，腎代替療法の導入時期の延長，腎代替療法治療様式の事前決定であった．その内容・実践は集団による講義形式のものが多く，病期に応じて複数のクラスを設けたり，プログラムのなかに個別対応する内容を組み込んで工夫しているものがあった．各文献では，患者への効果と費用削減の効果があったと述べられていたが，費用を具体的な数値を用いて報告していなかったり，算出項目に不足がみられた．患者教育プログラムの経済性を明らかにするには，介入効果と費用の把握を精確に行うことが重要で，経済的評価法を用いて研究することが必要である．

　【Purpose】 This study was intended to clarify the purposes, contents, and practices, efficacy for patients, and economical patient education programs to identify those appropriate for chronic kidney disease. 【Methods】 A literature review was conducted using the Internet edition of Igaku Chuo Zasshi, with the key words of“patient education”,“hemodialysis”, and“economics”all in Japanese, with the article type specified as“original paper”. A PubMed search was also performed using the key words of“patient education”,“renal dialysis”, and“economics”. Among the extracted papers, those describing the details of such programs were included in the review. 【Ethical consideration】 Not applicable as this study was a literature review. 【Results and Discussion】 While no economical patient education programs for chronic kidney disease were found in Japan, six reports on such programs were identified in foreign countries. The purposes of these programs included the practice and continuation of home hemodialysis, the postponement of the initiation of renal replacement therapy, and the choice of modality in renal replacement therapy ahead. Regarding the contents and practices, many programs were designed to be group session style, with some unique ones providing multiple classes corresponding to the disease stages, or incorporating tailormade program into a predefined program. Each paper claimed that the program had shown efficacy for patients as well as effectiveness in reducing costs, although some did not present the costs as specified amounts, while others lacked certain calculation items. Since an accurate understanding of the intervention effect and the cost is essential in clarifying the economical patient education program, it is necessary to perform the methods for the economic evaluation.
