本研究では、専門看護師による看護師を対象とした倫理研修プログラムを倫理的意識と行動の変化に着目し評価した。研修前後で質問紙調査を、3カ月後にフォーカスグループインタビューを行った。結果、「倫理への身近さ」「倫理カンファレンスへの参加意欲」「倫理カンファレンスの開催意欲」は有意に高まった(p<.05)。3カ月後、8名(44.4%)が倫理カンファレンスを開催していた。インタビューより意識の変化は【倫理の身近さへの気づき】【問題解決のための思考プロセスの理解】【視野の拡がりの実感】【倫理カンファレンスの有効性の実感】【倫理カンファレンス促進のための問題認識】の5カテゴリー、行動の変化は【必要な情報の収集】【スタッフへの教育的関わり】【他職種へのアプローチ】【カンファレンスの開催】【リソースの活用】の5カテゴリーを抽出した。本研修プログラムが倫理的意識を高め、臨床での行動の変化を促すことが示唆された。
In this study, we evaluated an ethics training programs for nurses by certified nurse specialists, focusing on changes in the nurses' ethical awareness and behavior. We administered an anonymous questionnaire survey before and after the training, and conducted a focus group interview 3 months later. As a result, “familiarity with ethics,” “motivation to participate in an ethics conference,” and “motivation to hold an ethics conference” were significantly higher (p<.05). After 3 months, 8 persons (44.4%) held the ethics conference. We extracted 5 categories of changes in awareness: awareness of the familiarity with ethics, understanding the thinking process for problem solving, feeling of expanding the field of view, feeling of the effectiveness of the ethics conference, and realizing the effectiveness of the ethics conference. We also identified 5 categories of changes in behavior from the interviews: awareness of the familiarity with ethics, understanding the thinking process for problem solving, feeling of expanding the field of view, feeling of the effectiveness of the ethics conference, and utilization of resources. These results suggest that the training programs were increase ethical awareness and promote behavioral changes in practice.
