要旨:身体機能障害に加え,アパシーによる意欲低下や抑うつ症状が出現した脳卒中患者を経験した.事例は介入初期より意欲低下を認め,具体的な目標設定が困難であった.また,経時的に抑うつ感・不安感が出現し,精神機能の評価結果が悪化した.これらの症状に対し,複数のポジティブな視覚フィードバックを用いて,身体機能に対する介入の効果が分かりやすくなるように提示した.結果,アパシーや抑うつの改善とともに言動変化を認め,具体的な目標共有が可能となった.本事例報告において,脳卒中後にアパシーや抑うつを呈した事例に対し,ポジティブな視覚フィードバックを用いた介入は,精神機能の改善や行動変容を促進する可能性が示された.
We experienced a stroke patient with physical dysfunction, apathy, low motivation, and depressive symptoms. The patient showed decreased motivation from the beginning of the intervention and had difficulty setting specific goals. Depression and anxiety appeared over time which worsened mental function assessment. To address these symptoms, multiple positive visual feedbacks were used to clearly present the effects of the intervention on physical function. As a result, the patient's behavior and speech changed, along with improvement in apathy and depression, and specific goals could be shared. This case study demonstrates that an intervention using positive visual feedback may promote improvement in mental function and behavioral change in a patient with apathy and depression after stroke.
