要旨：多発性脳梗塞により着衣障害を呈した65歳右利き女性の作業療法について報告する．本例は，視空間認知障害，視覚性運動失調，深部感覚障害を認め，着衣障害を呈していた．度重なる着衣の失敗経験から消極的となり，高次脳機能の検査評価を速やかに遂行できず難渋した．着衣動作観察と着衣プロセスの分析に従った系統的な評価を実施し，着衣プロセスの問題点に対応した代償的アプローチを行った．その結果，着衣障害の改善が見られた．本例では評価からアプローチまで着衣プロセスを利用することで，問題点の抽出とアプローチの相互関係の理解が容易となり，有効な代償的アプローチに結びついた可能性があると考えた．

　We report on the rehabilitation course of a 65-year-old, right-handed woman, who presented with a dressing disorder as a result of multiple brain infarctions. The patient's dressing disorder was related to visuospatial dysfunction, ataxie optique, and deep sensory disturbance. Because of frequent dressing failures, she became too apathetic to undergo detailed higher brain function assessments. Following a systematic evaluation of the patient's dressing process, a compensatory approach to the encountered problems was performed. Her dressing disorder improved through rehabilitation. Use of a common dressing format for evaluation and rehabilitation could be useful adjunct to the compensatory approach to treating dressing disorders. This format informs the rehabilitation approach and consistently extracts problems associated with dressing disorders.
