てんかん重積状態後出現した失行がてんかん外科と作業療法により改善した一例

Efficacy of surgical treatment and occupational therapy on status epilepticus-induced apraxia

要旨：てんかん重積状態後，左上肢に出現した失行に介入する機会を得た．本例は30歳代の女性で，てんかん重積状態を機に失行が出現するとともに発作頻度も増加したため，1年2ヵ月後にてんかん外科手術が実施された．術後，発作は抑制されたが失行は残存したため，リハビリテーションが指示された．作業療法開始時の所見では，左上肢運動麻痺は運動速度の低下のみで，物に対して腕や手のフォームを誤るという特性を有する失行を呈していた．模倣を用いた治療を実施した結果，短期間で改善を認めた．以上より，本症例の失行は重積を機に出現し，頻発するてんかん発作により継続した症状であると推定された．

　A 30-year-old woman with apraxia caused by status epilepticus underwent surgery for epilepsy 1 year and 2 months after the event. Although the epileptic seizure was controlled, apraxia resulted.

　Through neuropsychological evaluation the apraxia manifested itself through impaired reaching ability. Occupational therapy was adopted, and two weeks later, the symptoms improved.

　It is suggested that her apraxia was both caused and aggravated by frequent epileptic seizures.
