要旨:意味ある作業が健康に及ぼす影響についての根拠となる研究は少ない.本研究の目的は,意味ある作業の参加状況が健康関連QOLに及ぼす影響を明らかにすることである.茨城県の地域中高年者470名を対象に6ヵ月間の追跡調査を実施し398名を分析対象とした.意味ある作業の参加状況の測定は,自記式作業遂行指標(SOPI)を,健康関連QOLはSF-36v2を使用した.性・年齢・社会経済的要因を調整変数とした重回帰分析の結果,意味ある作業の参加状況の改善は健康関連QOL(特に精神的側面)に肯定的な影響があった.本研究は世界的にも少ない大規模集団の縦断研究であり,作業と健康に関する疫学的エビデンスの構築に貢献する.
There is little evidence that meaningful occupation has an important influence on health and well-being. Moreover, there are few longitudinal studies. Especially in Japan, there are only a few studies investigating the relationship between meaningful occupation and health and well-being. The purpose of this study was to examine the influence of participating in meaningful occupations on Health-Related Quality of Life (HR-QOL). We carried out a six-month follow-up survey for 470 community dwelling inhabitants over 50 years old in Ibaraki Prefecture, Japan. Of the 470 participants, 398 people were chosen as subjects (90 men, 308 women, mean age 63.2, range 50-79). The participation in meaningful occupations was measured by the Self-completed Occupational Performance Index (SOPI), and the HR-QOL was measured by the SF-36v2. The SOPI was based on the Canadian Model of Occupational Performance to measure the quality of participation in meaningful occupations and consisted of nine items. Through data analysis, a multiple regression analysis was carried out. We assumed the SOPI score as an independent variable and the SF-36v2 scores as dependent variables. The data analysis indicated that the change in the SOPI score during the six month period had a positive influence on Role Physical (RP), Bodily Pain (BP), General Health perceptions (GH), Vitality (VT), Social Functioning (SF), Mental Health (MH) and Mental Component Summary (MCS) of SF-36v2 after being adjusted for age, sex and socioeconomic factors. Therefore, we conclude that the improvement of the quality of participation in meaningful occupations had a positive influence on HR-QOL, particularly with regards to the mental aspect.
