　摂食・嚥下障害に対するアプローチにおいては，症状からその症例の問題点をとらえ，詳細な評価と分析から訓練手技を選択し，さらに修正を繰り返しながら訓練を実施していく．既存の訓練法でうまくいかないとき，さらに良い方法はないかと熟考を重ね，新たな発見や新しい訓練手技の開発につながることもある．K-pointは咬反射による開口障害患者に対して開口を促す部位として偶然発見したものだが，1症例報告に終わらせず臨床研究でEBMに結びつけ，嚥下訓練手技として発表してきた．今回のシンポジウムではこの経過を中心に，臨床研究の意義やEBMにもとづいた研究をするときの課題について報告した．

　In carrying out swallowing therapy, we should identify the problems from the patient's symptoms, select a treatment strategy through an in-depth diagnostic assessment, and administer treatment that is closely monitored and refined as necessary.　When no existing technique is effective, a new treatment technique may be developed after careful consideration.　I accidentally found the "K-point" to be the mouth-opening trigger point for a patient with hyperactive bite reflexes.　Continued clinical research has established the stimulation of the K-point as an evidence-based treatment method for patients with dysphagia.　In this paper, the importance of clinical research and some issues for conducting EBM research are presented.
