胸腔鏡下に修復したMorgagni孔ヘルニアの1例

Thoracoscopic repair of the Morgagni's hernia:A case report

　Morgagni孔ヘルニアは，胸骨後部より腹腔内臓器が胸腔内に逸脱する比較的まれな疾患である．無症状に経過することが多く，検診などにより胸部異常陰影を指摘され，発見されることが多い．術前のCT診断を応用し，胸腔鏡下にMorgagni孔ヘルニアを修復したので報告する．患者は43歳の女性で，4年前より右胸部異常陰影を指摘されており，陰影の増大にて当科に紹介された．胸腹部CT撮影後，MPR画像を作成し，Morgagni孔ヘルニアと診断した．ヘルニア門は右側のみで，ヘルニア内容は脂肪組織（大網）であることが確認され，胸腔鏡下手術を施行した．胸腔鏡により，胸腔内の癒着？離，ザックの処理が可能であった．

We recently experienced Morgagni's hernia who was a patient with diagnosed preoperatively by multiplanar reformation (MPR) CT.

A 43-year-old obese woman was referred to our hospital because of slow-growing abnormal shadow in the right cardiophrenic angle on chest roentgenogram.

Computed tomography (CT) demonstrated a mass of fat density behind the sternum and MPR-CT revealed a mass in the anterior thoracic cavity that went through Morgagni's foramen into the abdominal cavity.
