腹腔鏡下Morgagni孔ヘルニア修復術の経験

Laparoscopic repair of Morgagni hernia

　Morgagni孔ヘルニアに対して腹腔鏡下修復術を実施したので，文献的な考察も含めて報告する．症例は左胸部の不快感を有する69歳の女性で，画像診断によりMorgagni孔ヘルニアと診断された．肋弓下に3本のトロッカーを穿刺し，気腹下で腹腔鏡下手術を施行した．手術所見では，横隔膜下に存在する裂隙に横行結腸が嵌入していた．まず嵌入している横行結腸を腹腔内へ還納し，肝円索を切離した．ヘルニア嚢は切除せず，ヘルニア門の閉鎖のみを行った．2-0 Prolene^®を用いて横隔膜と腹直筋の後鞘を5針結節縫合し，さらに腹壁を貫通させた追加縫合を加えた．術後は症状も消失し，術後第5病日に退院となった．Morgagni孔ヘルニアに対する腹腔鏡下修復術は，従来の開腹下手術に代わり得る術式と考えられた．

A 69-year-old woman experienced discomfort in the right thoracic region. Based on the findings of chest X-ray, MRI, and Barium enema study, the patient was diagnosed with a Morgagni hernia and subjected to surgical operation. A laparoscopic repair of Morgagni hernia was performed using a subcostal insertion with three trocars. The hernia sac was not removed during laparoscopic herniorrhaphy in order to avoid pneumomediastinum.
