腹腔鏡下根治的膀胱摘除術と回腸導管造設術

Laparoscopic Radical Cystectomy and Heal Conduit

　浸潤性膀胱癌に対する根治的膀胱全摘術，回腸導管の腹腔鏡手術手技について述べた．膀胱前立腺摘除術に際しては，Denonvilliers腔に入り，前立腺尖部まで到達可能であることを確認することが重要と考えられ，膀胱子宮摘除術は腟前壁の切除に注意を要することで出血のコントロールをはかることができると考えられた．回腸導管は創外での腸管，尿管の切離，吻合を行っているが，さらに症例を重ねて術式の改良が望まれる．術後の飲食開始までの期間では差異はみられないものの，疼痛，歩行開始までの期間では開腹手術に比較して明らかに勝っており，低侵襲手術として有用と思われた．

We described the laparoscopic procedure of radical cystectomy and ileal conduit for invasive bladder cancer. In cystoprostatectomy, evaluating the plane between the prostate and the rectum is important in the operation. In female cystectomy, great care for the dissection of the anterolateral wall of the vagina minimized the blood loss. With further experience and improvement of operative procedure, laparoscopic radical cystectomy with urinary diversion may become one of the treatment options for patients with localized invasive bladder cancer.
