大腿型のsacless sliding fatty inguinal herniaに対してTEPを施行した1例

Laparoscopic totally extraperitoneal preperitoneal repair of a sacless sliding fatty inguinal hernia : A case report

◆要旨：症例は29歳，女性．右鼠径部の膨隆と疼痛を自覚して受診した．CT検査で脂肪の脱出した右大腿ヘルニアの診断で待機的に手術方針となった．腹腔鏡による観察では腹膜の陥凹を認めなかった．脂肪組織の脱出と考え，腹腔鏡下の腹膜前到達法によって大腿輪より脱出した脂肪組織を確認した．これを剝離して径1.0cmの筋膜欠損部を認めた．メッシュを用いてヘルニアを修復し，術後経過に問題なく退院した．術後約1年となるが，再発は認めていない．本邦ではsacless herniaの報告は少なく，新JHS分類（2021年版）上もヘルニア類似病変とされている．欧米ではsacless sliding fatty inguinal herniaの概念がある．sacless herniaに対する理解と適切な治療が重要であると考えられる1例を経験したため報告する．

　A 29-year-old female presented to our hospital with complaints of a mass and pain in her right groin. Abdominal computed tomography findings led to a diagnosis of a right femoral hernia with fatty tissues. Intraoperative findings revealed no peritoneal sac, suggesting the presence of a sacless hernia. Consequently, a totally extraperitoneal repair was performed. It was discovered that there was a slippage of preperitoneal fat which was resected to locate the hernial orifice. This report provides a detailed description of a case involving a sacless sliding fatty inguinal hernia.
