腹腔鏡観察下腹壁神経ブロックとその有用性

Transversus abdominis plane block（TAPB） and rectus sheath block（RSB） for post operative pain control after abdominal surgery

◆要旨：術後疼痛管理法として超音波ガイド下腹横筋膜面ブロック（TAPB），腹直筋鞘ブロック（RSB）が施行されている．通常，超音波ガイド下に経皮的に神経ブロックはされており，腹腔内臓器損傷など重篤な合併症が存在する．筆者らはTAPB，RSBを腹腔鏡手術の際に腹腔鏡下に行うことで，外科医でも安全に施行できると考えている．今回筆者らが腹腔鏡下手術の際に施行している末梢神経ブロックの手技について紹介する．

Transversus abdominis plane block（TAPB） and rectus sheath block（RSB） are associated with significant technical difficulties and risk of peritoneal organ perforation. So these techniques are usually limited to anesthesiologists. We report a much easier technique of performing RSB and TAPB during laparoscopic surgery. We think our technique may be done safely and easily.
