Reduced port surgeryによる腹腔鏡下膵体尾部切除術を施行した2例

Reduced port surgery for laparoscopic distal pancreatectomy : Report of two cases

◆要旨：単孔＋1ポートを用いたreduced port surgeryによる腹腔鏡下膵体尾部切除術を2例施行したので報告する．症例1：76歳女性，膵尾部に存在する微小浸潤膵管癌の疑い．症例2：53歳女性，膵尾部に存在する粘液性囊胞腫瘍の疑い．両症例ともに臍部にEZアクセスを使用した3本の5mmポート，左上腹部に12mmポートを挿入した単孔＋1ポートによる手術を施行した．Reduced port surgeryによる腹腔鏡下膵体尾部切除術は，当院で施行した従来の4ないしは5ポートによる同術式と比較しても手術成績は遜色なく，症例を選択することで妥当な術式になりうると考えられた．

Two cases of reduced port surgery（RPS） for laparoscopic distal pancreatectomy（Lap-DP） are reported. Case 1 : A 76-year-old woman visited our hospital complaining of dyspnea. Computed tomography showed stenosis and distal dilatation of main pancreatic duct in the pancreas tail. Minimally invasive pancreatic cancer was suspected, and RPS for Lap-DP was performed. Pathological diagnosis was minimal pancreatic cancer. Case 2 : A 53-year-old woman visited our hospital for treatment of pancreatic tail tumor. Magnetic resonance imaging showed a cystic tumor, measuring 40 mm, that contained mural nodule. RPS for Lap-DP was performed. Pathological diagnosis was pancreas neuroendocrine tumor. RPS for Lap-DP is a feasible procedure if the patient is selected carefully.
