Mini Loop RetractorⅡⓇを用いたグローブ法単孔式腹腔鏡下虫垂切除術

A new procedure for single incision laparoscopic appendectomy with Glove's method using Mini Loop RetractorⅡⓇ

◆要旨：当科では1997年にMini Loop RetractorⅡ^Ⓡ（MLR）を開発し，これを用いたreduced port surgeryを多数施行してきた．MLRは組織把持力・展開力が強く，炎症組織も損傷なく把持しうる．2009年6月からはMLRを用いたグローブ法単孔式腹腔鏡下虫垂切除術を考案し，定型化した．現在までに172例に施行し，手術時間50.5±17.6分，出血量全例で少量，術後在院日数3.57±2.73日，術後合併症4例（2.33％）と，良好な成績を得ている．当科の単孔式腹腔鏡下虫垂切除術は若手外科医にも安全に施行でき，標準手術として推奨できると考えられる．

We developed Mini Loop RetractorⅡ^Ⓡ（MLR）in 1997 and have experienced many reduced port laparoscopic surgery using this device. MLR is available with either a tight or loose grip. We can grip inflammatory tissue without damage using MLR. We started applying single incision laparoscopic appendectomy with the Glove's Method using MLR since June, 2009. Since then, a total of 172 patients underwent appendectomy by this method at our hospital. The mean operative time was 50.5±17.6 minutes and intraoperative blood loss was little. The mean postoperative hospital stay was 3.57±2.73 days. Postoperative complications were observed in 4 patients（2.33％）. Our method became a standard operation that can be performed by young surgeons, thus a useful method for appendectomy.
