近赤外線蛍光内視鏡イメージングシステムを利用した腹腔鏡手術前マーキングの経験

Preoperative colonic cancer tattooing using the near-infrared fluorescence laparoscopic imaging system

◆要旨：腹腔鏡下大腸癌手術における近赤外線蛍光内視鏡イメージングシステムを利用した術前マーキング法を経験したので報告する．手術前日から2日前に，内視鏡下に通常の点墨（0.2ml）に併せ腫瘍周囲の粘膜下層に0.25％ICG溶液を0.5ml局注した．術中，近赤外線蛍光内視鏡装置（PINPOINT System，NOVADAQ社製）を用いて，可視光モードおよび蛍光モードで，腹腔鏡下にマーキングされた腫瘍部位を観察した．可視光モードで点墨の視認が困難であった3例について報告する．蛍光モードでは全例ともICGを視認でき，腫瘍部位を確実に同定することが可能であった．PINPOINTはリアルタイムな腹腔鏡下手術の遂行も可能であり，次世代の大腸癌術前マーキング法・腹腔鏡ユニットとして期待される．

In colorectal cancer surgery, in order to identify the tumor site during surgery, preoperative endoscopic marking methods such as India ink tattooing or clipping methods are generally performed. However, there are problems in India ink tattooing method such as impaired surgical view due to adverse events, India ink leakage into the peritoneal cavity, and peritonitis or adhesion ileus as a result of ink leakage. For clipping method, it is necessary to check by palpation but this is not suitable in laparoscopic surgery. Previously, we devised a preoperative marking method for colorectal cancer surgery（laparotomy） by applying fluorescent properties of indocyanine green, and reported the usefulness of this method. This time, we report preoperative marking method that was applied to laparoscopic surgery.
