◆要旨：腹腔鏡下調節性胃バンディング術（LAGB）術後に発生したバンド逸脱に対して腹腔鏡下バンド除去および腹腔鏡下スリーブ状胃切除術（LSG）を施行した1例を経験したので報告する．患者は46歳，女性．2008年4月，LAGBを施行し，術後30kg以上の減量に成功していた．2012年1月，突然の嚥下障害を発症，消化管造影検査にてバンド逸脱を認め，再手術となった．腹腔鏡下に観察するとバンドに周囲臓器が癒着していた．癒着を丁寧に剝離してバンドを露出し，バンドを切離して抜去した．次いで自動縫合器を用いてLSGを施行した．LAGB後のrevisionの1つの術式としてLSGも有用と考えられ，若干の文献的考察を加え報告する．

We report a case of laparoscopic band removal and sleeve gastrectomy for band slippage after laparoscopic adjustable gastric banding（LAGB）. A 46-year-old woman underwent LAGB for morbid obesity on April 2008. After the operation, she lost more than 30kg in weight. However, she visited our hospital because of sudden onset dysphagia on January 2012. An upper gastrointestinal fluorography revealed the band slippage, and revision surgery was needed. Under the laparoscopy, adhesion between the band and surrounding tissues was carefully dissected and the band was cut and removed. Then sleeve gastrectomy was laparoscopically performed by endoscopic linear staplers. Laparoscopic sleeve gastrectomy is considered to be one of the feasible approaches to revision surgery for band slippage after LAGB.
