 One major problem in laparoscopic rectal surgery is that the small intestine descends into the pelvic cavity and the surgical field is interrupted. To cope with this problem, the patients are placed in a extreme head-down position, which frequently causes hemodynamic changes and brachial plexus paralysis as a less frequent but serious complication. We have developed a sponge made of dry compacted cellulose of which the volume can be increased approximately 9 times by soaking it in water. In laparoscopic rectal surgery in a pig, the sponge was inserted from a port, placed at the root of the mesentery, and swollen by physiological saline to displace the small intestine. With the aid of this method, we could complete laparoscopic rectal surgery, in the horizontal position while aguiring a good surgical field. After the procedure, the sponge could be easily removed from the abdominal cavity without making a new small laparotomy incision. Thus, we recommend this sponge device as a useful tool in laparoscopic surgery.
