 We performed laparoscopic cholecystectomy on a patient with a cholecystolithiasis in a left-sided gallbladder. The laparoscopic finding demonstrated that the gallbladder was located at the left side of the hepatic falciform ligament(left-sided gallbladder)and intraoperative cholangiography revealed that the cystic duct was connected to the left side of the common bile duct. MRI demonstrated the finding of the anomaly of intrahepatic portal vein and the left-sided gallbladder. When performing laparoscopic cholecystectomy for left-sided gallbladder, care should be taken to avoid the injury of the bile duct during the operation.
