Cチューブ留置法—Cチューブ開発の経緯と腹腔鏡下胆管切石術への応用

Technique of C Tube Placement: Details of C Tube and its Use in Laparoscopic Choledocholithotomy

　筆者は1981年，胆道手術におけるTチューブに替わる新しい方法として経胆嚢管チューブ（以下，Cチューブ）を開発した．Cチューブは胆嚢管から総胆管内に留置した細いチューブであるが，胆道系手術後の胆汁ドレナージのみならず，遺残結石の治療にも応用可能である．以後約250例の開腹手術に使用し，Tチューブに比べはるかに有用で合併症もなく，かつQOLを向上させることを明らかとしてきた．1991年から腹腔鏡下総胆管切石術に導入し，Cチューブの材質や腹腔鏡下での留置手技，チューブ固定法などに工夫，改良を加えてきた．本稿では，現在われわれが行っている腹腔鏡下Cチューブ留置法，および本法を用いた腹腔鏡下胆管切石術の手技を，術中写真を用いて詳述する．これまでの経験から，Cチューブ留置法は腹腔鏡下胆管切石術における標準手技となることを確信している．したがって，これまで述べてこなかったCチューブ開発の経緯についても述べ，Cチューブ留置法がより理解され，広く普及することを期待する．

We have developed a new technique for biliary surgery using the C tube that could replace the T tube in 1981. The C tube is a slender tube inserted via the cystic duct to the common bile duct. The placement of the C tube is used not only for postoperative bile drainage but also for the management of remnant stones. We have applied this C tube placement technique in more than 250 open surgical cases and demonstrated its validity and safety in comparison with the T tube. Since the introduction of C tube technique for laparos-copic surgery in 1991, we have tried to contrive and improve the C tube and its technique.
