There are few papers with good evidence on local burn treatment in Japan. In addition, the topical agents and wound dressings that can be used in Japan differ greatly from those used in other countries. In the treatment of the burned patient, it is important to first assess the depth of the burn appropriately and determine the appropriate treatment according to the depth of the burn. It is also important to determine whether the area of the burn affects the prognosis or not. This article introduces the literature and findings with a focus on severe and extensive burns requiring ICU management. The article first describes the types of topical medications and wound dressings, then discusses the use of honey, how to select topical medications and wound dressings according to guidelines, and the authors' thoughts regarding actual clinical practice. The insurance coverage period for wound dressings is limited to 2 weeks as the standard and 3 weeks when particularly necessary. However, it should be noted that there is a significant limitation in reimbursement practices for patients with severe and extensive burns that require daily dressing changes.
