Collaborative practice (CP), which follows interprofessional education, is the key stage and cornerstone of interprofessional collaboration. CP includes mechanisms such as institutional support, working culture, and environment. In CP, there are roles to be played not only by individual medical staff members but also by policy makers, hospital organizations, medical teams, etc., at each level, and CP is achieved when all of them continuously mesh, like cogs in a wheel. As medical providers, it is necessary to fill in the “gaps” in work, to coordinate the “overlap” of responsibilities and roles, and to create a “space” for collaboration with other professions. Each of these is not unique for individuals to work in a team, but it is important to clarify the issues and one's own role and tasks to achieve CP smoothly as a hospital system.
