Controversies remain regarding the pros and cons of protein restriction in critically ill patients with chronic kidney disease (CKD). However, in accordance with the hyperfiltration theory which is the mechanism of CKD progression, protein restriction is reasonable. Many clinical studies which suggest no restriction of protein exclude patients with CKD. Currently, it is considered reasonable to restrict protein at the time of admission to the intensive care unit, and then to adjust the amount given based on nitrogen balance. High quality randomized controlled trials are needed to examine the effectiveness of protein restriction in critically ill patients with CKD.
