Clinical examination and diagnosis for IgG4-related disease

IgG4関連疾患は，高IgG4血症と罹患臓器への著明なIgG4陽性形質細胞浸潤を特徴とする慢性炎症性疾患である．その診断には，罹患臓器の腫脹または肥厚を確認するための画像所見，上記血清学的所見および病理組織学的所見が重要となる．この病態では悪性腫瘍を除外することが必須であり，かつ同一個人に時間的・空間的に多発する合併症を認めることがあるため，全身を検索しなければならない．

IgG4-related disease is systemic and chronic inflammation, which is characterized by elevated level of serum IgG4 and abundant infiltration of IgG4-bearing plasmacytes into the organs involved.　There are important findings for the diagnosis ; the enlargement or thickness of the organs, and the above serological and pathological findings.　At the same time, it is necessary that malignant neoplasms should be excluded.　We also have to evaluate other organs systemically because multiple complications, which occur temporally and spatially in the same patient, are also observed.
