Sources of strong power such as nuclear energy should be properly controlled under safety, risk, & quality management systems. In this day and age when medical technology has a powerful impact on the human body in ways good or bad, it should also be more precisely controlled than ever under these systems. In the last decade, we aimed to create such management and educational systems at Tsukuba University Hospital. Our final goal is the ongoing improvement of the overall quality of medical services provided under these systems, where all medical incidents, occurrences and accidents are considered as defects in quality, and every process of medical procedure is reviewed and revised by professionals with expert knowledge of such incidents (by the method we called the evidence-based PDCA cycle).
