The diagnosis of primary osteoporosis is made using conventional radiology of the spine and by measuring the bone mineral density (BMD). BMD should be measured basically at the lumbar spines. Bone metabolism markers are useful in evaluating bone metabolism. Patients with increased bone resorption are indicated for treatment by antiresorptive agents. Bone metabolism markers are also useful for the judgement of treatment success and switching to other treatment regimens. Estrogen deficiency following menopause is correlated with a rapid reduction in BMD. After menopause, bone remodeling increases. At the same time, however, bone resorption exceeds bone formation, resulting in osteoporosis.
