Diagnosis for osteopenia and osteoporosis

原発性骨粗鬆症の検査には，診断のための検査として脊椎のレントゲンと骨密度測定があり，また，続発性骨粗鬆症の鑑別診断としての検査や，骨代謝を評価するための骨代謝マーカーの測定などがある．骨密度測定は原則として腰椎骨密度で評価するが，骨折などで評価できない場合は大腿骨頸部やそのほかの末梢骨でもよい．鑑別診断には詳細な問診や身体所見のほか，血清カルシウムやリン，ALP(alkaline phosphatase)などの測定が役に立つ．骨吸収が亢進しているときには，ビスホスホネートなどの骨吸収抑制剤のよい適応となる．またその後の治療効果判定や薬剤変更などにも骨代謝マーカーが参考になる．閉経後は骨代謝が亢進するが，骨吸収優位のため骨密度は減少していく．

The diagnosis of primary osteoporosis is made using conventional radiology of the spine and by measuring the bone mineral density (BMD).　BMD should be measured basically at the lumbar spines.　Bone metabolism markers are useful in evaluating bone metabolism.　Patients with increased bone resorption are indicated for treatment by antiresorptive agents.　Bone metabolism markers are also useful for the judgement of treatment success and switching to other treatment regimens.　Estrogen deficiency following menopause is correlated with a rapid reduction in BMD.　After menopause, bone remodeling increases.　At the same time, however, bone resorption exceeds bone formation, resulting in osteoporosis.
